{"id":2360,"date":"2016-11-06T11:25:45","date_gmt":"2016-11-06T11:25:45","guid":{"rendered":"https:\/\/armando.info\/?p=2360"},"modified":"2016-11-06T11:25:45","modified_gmt":"2016-11-06T11:25:45","slug":"the-silent-spread-of-diphtheria","status":"publish","type":"post","link":"https:\/\/armando.info\/en\/the-silent-spread-of-diphtheria\/","title":{"rendered":"The Silent Spread of Diphtheria"},"content":{"rendered":"\n<p class=\"wp-block-paragraph\">Diphtheria, \r\nthe contagious disease eradicated 24 years ago in Venezuela, has reborn in \r\nBol\u00edvar state (south of the country). Epidemiologists presume that the outbreak \r\nhas arisen in an indigenous community, San Antonio Rosc\u00edo, and has spread since \r\nApril through other towns. At Hospital Ra\u00fal Leoni -known as Guaiparo- arrive \r\nsuspicious cases from the states of Anzo\u00e1tegui, Bol\u00edvar and Delta Amacuro. \r\n\"Runny nose, swollen tonsils, signs like a cold. It could be confused with \r\ntonsillitis, but nobody dies of that. So when death comes, we suspect that it is \r\ndiphtheria,\" says a doctor.<\/p>\n\n\r\n\n<p class=\"wp-block-paragraph\">Eight-year-old \r\nAbismael Espinoza Rodr\u00edguez arrived at the Guaiparo hospital with this clinical \r\ncondition, one night in late September. The parents of the child had gone first \r\nto a medical module of Barrio Adentro \u2014the health program sponsored by former \r\nPresident Hugo Ch\u00e1vez with Cuba\u2019s cooperation\u2014 in the town of San F\u00e9lix. \"The \r\ndoctor said it was tonsillitis, so we went back home and just follow the \r\ntreatment. But as days passed, he got worse. We decided to take him to a \r\nhospital and that's when the worst happened,\" says Gregoria Rodr\u00edguez, the boy's \r\nmother.<\/p>\n\n\r\n\n<p class=\"wp-block-paragraph\">On \r\nOctober 11, the Minister of Health, Luisana Melo, spoke for the first time about \r\nthe outbreak of the disease: \"It is a regional issue, in Bol\u00edvar state only. Of \r\nall the suspected cases, four cases of diphtheria were found and two confirmed \r\ndeaths. \"But 12 days before these statements, there was another government \r\ndiagnosis. Like his, the death certificates of three other \r\nchildren from Bol\u00edvar - Sara Bastardo, Jhoangely Alexandra Ochoa, and Dariervys \r\nBraz\u00f3n -, issued between September 24 and October 10, and obtained by \r\nArmando.info, indicated diphtheria as the cause the deaths. For physicians in \r\nthe health centers of Bol\u00edvar, witnesses of the resurgence of diphtheria, an \r\nepidemic is hidden. \"The confirmation of cases is usually delayed, because \r\nalmost all of the evidence is sent to Caracas. However, we already have several \r\npatients, dozens, and they do not know each other. There are more than 20 deaths \r\nfrom this disease. Even the pediatric ward of the hospital (Ra\u00fal Leoni) has been \r\nenabled to attend only potential patients of diphtheria,\" says a doctor at this \r\nhospital.<\/p>\n\n\r\n\n<p class=\"wp-block-paragraph\">Diphtheria \r\nis a preventable disease. The expedited way to avoid an outbreak in the \r\npopulation is vaccination. In Venezuela, immunization depends on health programs \r\nrun by the State. According to recommendations of the World Health Organization \r\n(WHO), countries should immunize 90 percent of the population, in this case with \r\nthe vaccine known as DPT3, to prevent the re-emergence of this disease. However, \r\nthis guideline has not been complied with in Bol\u00edvar state in the last \r\ndecade.<\/p>\n\n\n<figure class=\"wp-block-pullquote alignleft\"><blockquote><p>It is not a regional problem, as the minister affirms<\/p><\/blockquote><\/figure>\n\n\n<p class=\"wp-block-paragraph\">The \r\nparents of Abismael Espinoza do not remember if the child got the three doses of \r\nthe vaccine against diphtheria. In 2008, the year in which Espinoza was born, \r\ncoverage of the DPT3 vaccine in Bol\u00edvar state reached 16.5 percent, according to \r\nrecords of the Yearbook of the Expanded Program on Immunizations (1962-2009) of \r\nthe Ministry of Health. The figures from entities have not been disclosed \r\nperiodically in the last six years, although the Venezuelan Society of Public \r\nHealth has got the feel of the epidemiological situation based on official and \r\nother leaked statistics. \"Bol\u00edvar is one of the six most vulnerable states in \r\nVenezuela, because until mid-2016 the immunization coverage against this disease \r\nwas at an annual rate of 42 percent. Other states in similar conditions are \r\nBarinas, Carabobo, Delta Amacuro, Monagas and Zulia. This reveals that it is not \r\na regional problem, as the minister points out. It is already an epidemic, a \r\nsingle case is enough to be considered an epidemic, and it can expand to other \r\nplaces,\" says Jos\u00e9 F\u00e9lix Oletta, a member of this organization and former \r\nMinister of Health.<\/p>\n\n\r\n\n<p class=\"wp-block-paragraph\">Until \r\na few days ago, there were no evident signs of health prevention. At the airport \r\nof Puerto Ordaz, in Bol\u00edvar, and at the main land terminals, the health \r\nconditions of the passengers were not examined. In contrast, the outbreak is \r\nconsidered a danger abroad, although the Pan American Health Organization (PAHO) \r\nhas not yet issued an alert. In Guyana, which borders Venezuela precisely on the \r\nborder of Bol\u00edvar state, they have implemented an intense vaccination campaign \r\nto prevent the spread of the disease, while the Ministry of Public Health of the \r\nDominican Republic has recommended the medical staff of the International \r\nAirport of Las Americas to be vigilant with travelers from Venezuela in the face \r\nof the resurgence of diphtheria.<\/p>\n\n\n<figure class=\"wp-block-image size-medium\"><img loading=\"lazy\" decoding=\"async\" width=\"640\" height=\"547\" src=\"https:\/\/armando.info\/wp-content\/uploads\/2020\/12\/06112016-3.jpg\" alt=\"\" class=\"wp-image-51439\"\/><figcaption>Espromed Bio C.A. fa\u00e7ade. Photo: Ministry of People\u2019s Power for Health Press<\/figcaption><\/figure>\n\n\n<h2 class=\"wp-block-heading\">The Distant Antidote<\/h2>\n\n\n<p class=\"wp-block-paragraph\">The \r\nparents of the four children whose death certificates indicate that they died of \r\ndiphtheria admitted that they did not completed the required doses of the \r\nvaccines. \"I always wanted to vaccinate my baby, but when I went to the \r\noutpatient clinic or the hospital, they told me that the vaccine had run out. At \r\nsome point, I threw in the towel and did not keep going,\" says \r\nMelville.<\/p>\n\n\r\n\n<p class=\"wp-block-paragraph\">Diphtheria \r\nvaccines depend on an agreement signed between Cuba and Venezuela, through the \r\nSocialist Company for the Production of Biological Medicines, Espromed Bio C.A., \r\ncreated on June 12, 2014 by a decree of President Nicol\u00e1s \r\nMaduro.<\/p>\n\n\r\n\n<p class=\"wp-block-paragraph\">However, \r\nEspromed Bio was not an invention of the Government. The vaccine plant, in \r\nUniversidad central de Venezuela, already existed since 2000 and depended on the \r\nRafael Rangel National Hygiene Institute. The Government only decided to \r\ntransfer the administration directly to the Ministry of Health to reinforce its \r\noperation. The company was expected to meet the country's demand, since it is \r\nmade up of three independent laboratories for the industrial fermentation \r\nprocess with the capacity to produce 160 million doses of diphtheria toxoid, \r\nanti-Pertussis vaccine and tetanus toxoid per year, and with a plant of \r\nformulation, filling and final processing, with an installed capacity of 120 \r\nmillion doses per year.<\/p>\n\n\r\n\n<p class=\"wp-block-paragraph\">The \r\nemerging promise has not escaped from the Venezuelan decline though. Last year, \r\nEspromed Bio only produced the packaging and labeling of 21,718,906 doses of \r\nbiological products and essential medicines to prevent diseases caused by \r\nviruses, bacteria, toxins, among other agents. Only 5,104,842 out of this amount \r\n\u2014almost all the doses acquired from the agreement between Cuba and \r\nVenezuela\u2014were distributed in the country. According to the 2015 Annual Report \r\nof the Ministry of Health, the company was victim of delays in the allocation of \r\nforeign currency to import spare parts and supplies for production, among other \r\nadministrative obstacles.<\/p>\n\n\r\n\n<p class=\"wp-block-paragraph\">The \r\nreappearance of diphtheria has coincided with the worst health crisis faced in \r\nVenezuela. The country, dependent on oil and subject to strict foreign exchange \r\ncontrol, has fallen into a financial meltdown that causes a drop in imports. \r\nThis situation has affected hospitals, pharmacies and laboratories that, in most \r\ncases, are supplied with inputs acquired abroad. It is estimated that there is \r\nalmost a deficit of 85 percent of drugs -according to the Venezuelan \r\nPharmaceutical Federation- and a growing deterioration of medical centers. But \r\nthe Government of Maduro has coined the term of \"economic war\" to hold it \r\nresponsible for all ills.<\/p>\n\n\n\t\t\t\t<figure class=\"wp-block-embed is-type-rich is-provider-embed-handler wp-block-embed-embed-handler wp-embed-aspect-16-9 wp-has-aspect-ratio\"><div class=\"wp-block-embed__wrapper\">\n\t\t\t\t<iframe loading=\"lazy\" title=\"Ministra de Salud desmiente casos de Difteria\" width=\"500\" height=\"281\" src=\"https:\/\/www.youtube.com\/embed\/036PWXQkaj4?feature=oembed\" frameborder=\"0\" allow=\"accelerometer; autoplay; clipboard-write; encrypted-media; gyroscope; picture-in-picture; web-share\" referrerpolicy=\"strict-origin-when-cross-origin\" allowfullscreen><\/iframe>\n\t\t\t\t<\/div><\/figure>\n\t\t\t\t\n\t\t\t\t \n\n<h2 class=\"wp-block-heading\">The Gospel of Omission<\/h2>\n\n\n<p class=\"wp-block-paragraph\">In \r\nthe Christian church of the Buen Retiro neighborhood of San Felix, in the heart \r\nof Bol\u00edvar state, they have said a prayer for the soul of Sara\u00ed Bastardo. The \r\ninhabitants of this walled village still cannot find explanations for the sudden \r\ndeath of the girl. Solmaira Melville, the mother of Sara\u00ed, resents not having \r\nnoticed in time the signs of faintness of the daughter: \"Dyspnea, sore throat, \r\nstifling sensation in the chest. Since she was asthmatic, we thought that was \r\nwhat she had, and that she would improve with medication. But it was not that, \r\nit was diphtheria.\"<\/p>\n\n\r\n\n<p class=\"wp-block-paragraph\">The \r\noutbreak of diphtheria has unleashed fear in Bol\u00edvar. Melville denounces that \r\none of his three children has been isolated at school. \"A teacher began to say \r\nthat the child could not be with the other children; that he had to be subject \r\nto medical exams before rejoining. After the death of my baby, we have all been \r\nvaccinated. Already, after several complaints, the principal is aware of this \r\nand allowed the child to attend school,\" she says.<\/p>\n\n\r\n\n<p class=\"wp-block-paragraph\">In \r\nthe neighborhood of the Espinoza Rodriguez family, the mourners of Abismael, is \r\nalready aware of the diphtheria due to the death of the child. \"Ask about the \r\nmother of the child who died of diphtheria! Anyone can tell you where my home \r\nis,\" says Gregoria Rodr\u00edguez, when scheduling a journalistic \r\ninterview.<\/p>\n\n\n<figure class=\"wp-block-pullquote alignleft\"><blockquote><p>The worst recipe applied by the Ministry of Health is \"concealment\" of information<\/p><\/blockquote><\/figure>\n\n\n<p class=\"wp-block-paragraph\">According \r\nto Oletta, the worst recipe applied by the Ministry of Health is \"concealment\" \r\nof information. \"Minister Melo has made a serious mistake. The worst that can be \r\ndone in an epidemic is to say nothing or that everything is under control when \r\nit is not. In the end, that delegitimizes her. It also causes discrimination \r\nwithin the population due to uncertainty. Damage is being caused in the country \r\ndue the bad policies and for not having the courage to rectify and admit that \r\nthey have been wrong,\" he explains.<\/p>\n\n\r\n\n<p class=\"wp-block-paragraph\">Since \r\nthe health authority gave a statement on October 11, no more official figures on \r\nthe outbreak have been released. Armando.info contacted the Ministry of Health \r\nto obtain updated statistics on diphtheria, but they responded that it was not \r\npossible to know them because Melo was in the presentation of Venezuela\u2019s Human \r\nRights report before the United Nations Organization in Geneva (Switzerland), \r\nand other spokespersons had to be authorized to give a \r\nstatement.<\/p>\n\n\r\n\n<p class=\"wp-block-paragraph\">The \r\nsilence of the Ministry of Health is questioned. This week, Melo received strong \r\ncriticism on Twitter, to the point of becoming a trend in the social network, by \r\nomitting the health crisis in Venezuela during her speech to the United Nations \r\nin Geneva, in the second Universal Periodic Review. She said that the government \r\nhas distributed over 6.5 million vaccines to protect Venezuelans from 16 \r\ndifferent types of diseases, but she failed to talk about diphtheria. There is \r\nalso no information on the outbreak in the south of the country in the portal of \r\nhealth or in its social networks. The State apparatus continues admitting only \r\ntwo deaths caused by this disease, although at least twice as many deaths could \r\nbe confirmed for this report.
